Personal Information
While you need not provide us with any personal information, there are times, however, when we may need information from you, such as your name, telephone number and e-mail address, to correspond with you or facilitate your electronic request for an appointment, for example.  If you choose to give us personal information via the Internet, it is our intent to let you know how we will use such information to contact you.  If you tell us that you do not wish to have this information used as a basis for further contact with you, we will respect your wishes.

Marketing E-mails
If you receive a marketing e-mail from Recovery Physical Therapy, your e-mail address is listed with us as someone who has expressly shared that address for the purpose of requesting an appointment, receiving information in the future or you otherwise have an existing relationship with us.

We will not share, sell or rent your personal information without your permission or unless ordered by a court of law.

The information you submit to us is available only to employees who manage this information for the purposes of facilitating your request for an appointment or for sending you marketing e-mail communications and to contracted service providers for the purpose of providing services related to our communications with you.

We use security measures to protect against the loss, misuse or alteration of your information.

Please note that if you request an appointment with Recovery Physical Therapy or reply to a Recovery Physical Therapy address in one of our marketing e-mails, the communication you send to us will not create a physical therapist-patient relationship with us.  Do not send us any medical or other information that you or anyone else considers to be confidential or private unless we have first agreed to be your physical therapist.  Any information you send us before we agree to be your physical therapist cannot be protected from disclosure.
